    目的 探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清低氧诱导因子1α(HIF-1α) 和血管内皮生长因子(VEGF)水平及其与血压的关系。方法 选取2013 年1 月-2016 年10 月经多导睡眠图 (PSG)检测诊断为OSAHS 患者80 例作为研究组,同期选取健康体检者50 例作为对照组。均行24 h 动态血 压(ABP)监测,分析日、夜间平均收缩压(dSBP、nSBP)和日、夜间平均舒张压(dDBP、nDBP)。采用酶联免 疫吸附法试剂盒检测所有入选者血清HIF-1α、VEGF 水平,并对OSAHS 患者的睡眠呼吸暂停低通气指数 (AHI)、血清HIF-1α、VEGF 水平及血压间进行相关性分析。结果 OSAHS 组血清HIF-1α 和VEGF 水平 均高于对照组(P <0.05),且升高水平与AHI 程度呈正相关(P <0.05);OSAHS 组的nSBP、nDBP 也均高于 对照组(P <0.05),而dSBP、dDBP 的比较,差异无统计学意义(P >0.05)。血清HIF-1α 和VEGF 水平分 别与nSBP、nDBP 均呈正相关(P <0.05)。结论 OSAHS 患者血清HIF-1α 和VEGF 水平的高表达可能是影 响患者夜间血压升高的危险因素。

    Abstract:

    Objective To investigate the relationship between hypoxia inducible factor-1α (HIF-1α), vascular endothelial growth factor (VEGF) and blood pressure in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods The survey was conducted in 130 patients selected from the special care medical center of Naval General Hospital of PLA from January 2013 to October 2016. The ambulatory blood pressure monitoring (ABMP) and polysomnography (PSG) were determined. According to the result of PSG test, 130 subjects were divided into obstructive sleep apnea hypopnea syndrome group (n = 80) and normal control group (n = 50). The level of serum HIF-1α and VEGF were detected by ELISA method. The parameters of the blood pressure were compared in the two groups and the relationship between HIF-1α, VEGF, apnea hypoventilation index (AHI) and blood pressure were comparatively analyzed in obstructive sleep apnea hypopnea syndrome group. Results For the patients with obstructive sleep apnea hypopnea syndrome, the levels of HIF-1α and VEGF were significantly higher than those of normal control group (P < 0.05), they were positively correlated with AHI (P < 0.05). The night mean systolic blood pressure (nSBP) and night mean diastolic blood pressure (nDBP) were significantly higher in obstructive sleep apnea hypopnea syndrome group (P < 0.05). Conclusion HIF-1α and VEGF may be risk factors which can influence night blood pressure in patients with obstructive sleep apnea hypopnea syndrome.
