    目的 探讨聚焦超声联合竹红菌素软膏治疗对外阴上皮非瘤样病变组织P16、CD34 的影响。方法 选取2010 年12 月-2015 年12 月在衡水市人民医院进行治疗的80 例外阴上皮非瘤样病变(NNED)患者为 研究对象。其中,2013 年12 月以前入院的40 例患者作为对照组,选择与其状况相似的2013 年12 月以后入院 的40 例患者作为观察组。对照组患者接受聚焦超声治疗,观察组患者接受聚焦超声联合竹红菌素治疗。比 较两组患者治疗效果和治疗前后病变组织P16、CD34 水平。结果 治疗后两组患者外阴瘙痒评分、皮肤弹性 评分和外阴白色病变面积均变小(P <0.05),观察组患者外阴瘙痒评分、皮肤弹性评分和外阴白色病变面积 小于对照组(均P <0.05)。治疗后两组患者外阴组织P16 蛋白水平均上升(P <0.05),但观察组患者治疗后 P16 水平较对照组更高(P <0.05),CD34 蛋白水平下降(P <0.05),但观察组患者治疗后CD34 蛋白水平更低 (P <0.05)。结论 聚焦超声联合竹红菌素软膏治疗NNED 疗效好,能增加病变组织中P16 蛋白含量,降低CD34 蛋白含量,提示NNED 的发生发展可能与细胞周期失调和微循环障碍有关。

    Abstract:

    Objective To explore therapeutic effect of focused ultrasound combined with Hypocrellins in patients with non-neoplastic intraepithelial lesions of vulva. Methods Totally 80 cases of non-epithelial tumor like lesions of epithelium were admitted in our hospital from December 2010 to December 2015. All patients received focused ultrasound treatment as standard treatment. Patients in observation group received additional treatment of Hypocrellins. Levels of P16 and CD34 in tissues were measured. Results Treatments induced decreased levels of scores of vulvar pruritus, skin elasticity score and area of white lesions of vulva in the two groups; additional treatment of Hypocrellins in observation group induced more significant improvement when compared with control group (P < 0.05). All patients experienced increased levels of P16 protein and decreased levels of CD34 protein compared with those prior to any treatments. Still, treatment with Hypocrellins induced dramatical increase of P16 and decrease of CD34 compared with control group. Conclusions Focused ultrasound combined with Hypocrellins exerts promising therapeutic effect in treatment of NNED through manipulating cell cycle and microcirculation.
