通气散联合阿莫西林克拉维酸钾对 急性化脓性中耳炎的疗效分析

Effect of tongqi powder combined with amoxicillin potassium clavulanate on acute suppurative otitis media

摘要:

目的 探讨通气散联合阿莫西林克拉维酸钾对急性化脓性中耳炎（APOM）的疗效。方法 选 取2016 年6 月—2018 年6 月宝鸡市中心医院收治的84 例APOM 患者，采用随机数字表法分成观察组和对照 组，每组42 例。对照组口服阿莫西林克拉维酸钾片；观察组口服通气散联合阿莫西林克拉维酸钾。比较两组 临床疗效、听力阈值、致病菌清除及免疫功能与炎症因子情况。结果 观察组治疗总有效率高于对照组（97.6% VS 85.7%）（P <0.05）；两组患者治疗后听力阈值和致病菌降低（P <0.05），且观察组较对照组低（P <0.05）； 两组患者治疗后外周血CD4+、CD8+ 及CD4+/CD8+ 较治疗前下降（P <0.05），且观察组低于对照组（P <0.05）。 两组患者外周血白细胞介素-2（IL-2）、白细胞介素-4（IL-4）及白细胞介素-6（IL-6）比较，差异有统 计学意义（P <0.05）；两组治疗后外周血IL-2、IL-4 及IL-6 较治疗前下降（P <0.05）。结论 通气散联合 阿莫西林克拉维酸钾治疗APOM 可清除致病菌、改善听力，从而提高临床疗效；同时还可改善患者免疫功能， 降低中耳炎症反应。

Abstract:

Objective To explore the clinical effect of tongqi powder combined with amoxicillin potassium clavulanate on acute suppurative otitis media. Methods Eighty-four patients with APOM admitted to our hospital from June 2016 to June 2018 were selected and divided into observation group and control group by random number table method, with 42 cases in each group. The control group was given oral amoxicillin potassium clavulanatetablets, and the observation group was given oral tongqi powder combined with amoxicillin clavicylate potassium tablets. The clinical efficacy, hearing threshold, pathogen clearance, immune function and inflammatory factors of the two groups were compared. Results After treatment, the total clinical effective rate of observation group was 97.6%, which was higher than that of control group (85.7%), and the difference was statistically significant (P < 0.05); the hearing threshold and pathogenic bacteria in both groups were significantly reduced, and observation group was lower than control group (P < 0.05); the percentages of CD4+, CD8+ and CD4+ /CD8+ in the two groups all decreased compared with the previous group, but observation group was lower than control group (P < 0.05); the levels of IL2, IL4 and IL6 in peripheral blood serum of the two groups all decreased compared with the previous group, but there were significant differences between two groups (P < 0.05). Conclusions Tongqi powder combined with amoxicillin potassium clavulanate in the treatment of acute purulent otitis media can not only eliminate pathogenic bacteria, improve hearing and thereby improve clinical efficacy, but also improve the immune function of patients and reduce the middle ear inflammatory response.
