眉下切口提眉术联合重睑成形术治疗上睑皮肤松弛对眼眶形态改善效果及并发症的影响

Effect of sub-brow incision lifting combined with double eyelid surgery on orbital morphology improvement and complications in the treatment of upper eyelid skin laxity

摘要:

目的 探究眉下切口提眉术（SBIL）联合重睑成形术治疗上睑皮肤松弛对眼眶形态改善及并发症的影响。方法 选取2022年3月—2024年3月三亚市中医院和洛阳市西工莱尔美医疗美容诊所就诊的100例上睑皮肤松弛患者为研究对象，按手术方法分为SBIL组（48例）与联合组（52例）。SBIL组给予SBIL治疗，联合组给予SBIL联合重睑成形术治疗。比较两组眼眶形态指标、Lemperle评分、视觉模拟评分法（VAS）评分、满意度及并发症。结果 联合组术后睑缘-角膜映光点距离（MRD）、角膜内侧睑缘与重睑褶皱距离（MCMFD）、瞳孔中点睑缘与重睑褶皱距离（MPMFD）、外眦处睑缘与重睑褶皱距离（LCMFD）均大于SBIL组（P <0.05）；联合组治疗前后MRD、MCMFD、MPMFD、LCMFD的差值均大于SBIL组（P <0.05）。术后1、2个月，联合组的Lemperle评分均低于SBIL组（P <0.05）。联合组术后患者VAS评分、医师VAS评分均高于SBIL组（P <0.05）；联合组治疗前后患者VAS评分、医师VAS评分的差值均大于SBIL组（P <0.05）。联合组的总满意度高于SBIL组（P < 0.05）；联合组的并发症发生率与SBIL组比较，差异无统计学意义（P >0.05）。结论 SBIL联合重睑成形术治疗上睑皮肤松弛对患者眼眶形态改善及并发症有较为显著的影响。

Abstract:

Objective To explore the effects of sub-brow incision lifting (SBIL) combined with double eyelid surgery on the improvement of upper eyelid skin laxity and orbital morphology, as well as the incidence of complications.Methods A total of 100 patients with upper eyelid skin laxity who were admitted to Sanya Traditional Chinese Medicine Hospital and Luoyang Xigong Laiermei Medical Beauty Clinic in Henan Province from March 2022 to March 2024 were selected for this study. The patients were divided into two groups according to the surgical method: the SBIL group (n = 48) received SBIL treatment, and the combined group (n = 52) received SBIL combined with double eyelid surgery. The outcomes compared between the two groups included orbital morphology indicators, lemperle scores, visual analogue scale (VAS) scores, satisfaction levels, and incidence of complications.Results The postoperative marginal reflex distance (MRD), medial canthal margin to fold distance (MCMFD), mid-pupillary margin to fold distance (MPMFD), and lateral canthal margin to fold distance (LCMFD) in the combined group were all significantly higher than those in the SBIL group (P < 0.05). The pre-to-postoperative changes in MRD, MCMFD, MPMFD, and LCMFD before and after treatment were also significantly greater in the combined group compared to the SBIL group (P < 0.05). At 1 and 2 months postoperatively, the lemperle scores in the combined group were significantly lower than those in the SBIL group (P < 0.05). Postoperative patient-rated and physician-rated VAS scores were both significantly higher in the combined group than in the SBIL group (P < 0.05), and the changes in patient-rated and physician-rated VAS scores from before to after treatment were also significantly greater in the combined group (P < 0.05). The overall satisfaction in the combined group was higher than in the SBIL group (P < 0.05). The incidence of complications between the combined group and the SBIL group showed no statistically significant difference (P > 0.05).Conclusion SBIL combined with double eyelid surgery has a significant effect on improving orbital morphology, with a comparable safety profile in terms of complications.
