Abstract:Objective To investigate the associations between serum levels of netrin-1 and α-synuclein (α-Syn) and cognitive function in patients with first-episode schizophrenia (SCZ).Methods A total of 120 patients with first-episode SCZ admitted to Hangzhou Seventh People's Hospital between September 2023 and September 2025 were selected as the study group, whilst 113 individuals undergoing health check-ups at the same hospital during the same period were selected as the control group. The demographic characteristics, serum netrin-1 and α-Syn levels, and MATRICS Consensus Cognitive Battery (MCCB) scores were compared between the control and study groups. Pearson’s correlation analysis was performed to assess the relationship between serum netrin-1 and α-Syn levels and cognitive function scores in the study group.Results There were no statistically significant differences between the control and study groups in terms of sex distribution, age, body mass index, smoking history, alcohol consumption history, educational background, family history of mental illness, prevalence of hypertension, prevalence of diabetes mellitus, prevalence of hyperlipidemia, systolic blood pressure, diastolic blood pressure, fasting blood glucose, or triglyceride, total cholesterol, high-density lipoprotein cholesterol, and low-density lipoprotein cholesterol levels (P > 0.05). Netrin-1 levels were lower in the study group than in the control group, whilst α-Syn levels were higher in the study group (P < 0.05). The scores of all domains of the MCCB, including speed of processing, working memory, verbal learning, visual learning, reasoning and problem solving, attention/vigilance, and social cognition, were significantly lower in the study group than in the control group (P < 0.05). Pearson correlation analysis indicated that netrin-1 levels in the study group were positively correlated with scores of all domains of the MCCB (P < 0.05), whilst α-Syn levels were negatively correlated with all these scores (P < 0.05).Conclusion Patients with first-episode SCZ have reduced serum netrin-1 levels and elevated α-Syn levels, and both of them are associated with cognitive impairment in patients.
