独活寄生汤联合玻璃酸钠关节腔注射对肝肾不足型膝关节骨关节炎的临床观察

Clinical observation of Duhuo Jisheng Decoction combined with sodium hyaluronate intra-articular injection for knee osteoarthritis with liver and kidney deficiency

摘要:

目的 探究独活寄生汤联合玻璃酸钠关节腔注射在肝肾不足型膝关节骨关节炎中的应用效果。方法 回顾性选取2023年4月—2025年4月西安市第九医院收治的60例肝肾不足型膝关节骨关节炎患者，按治疗方法分为单一组和汤剂联合组，每组30例。单一组给予玻璃酸钠关节腔注射治疗，汤剂联合组在单一组的基础上联合独活寄生汤治疗。比较两组临床疗效、中医证候积分、膝关节功能、视觉模拟评分量表（VAS）评分及炎症因子。结果 汤剂联合组的临床疗效总有效率高于单一组（P <0.05）。治疗后，汤剂联合组中医证候积分低于单一组（P <0.05）；汤剂联合组的Lysholm膝关节功能评分、美国特种外科医院膝关节评分高于单一组，西安大略和麦克马斯特大学骨关节炎指数低于单一组（P <0.05）。治疗3、5周后，汤剂联合组的VAS评分均低于单一组（P <0.05）。治疗后，汤剂联合组的C-反应蛋白、白细胞介素-17、白细胞介素-8均低于单一组（P <0.05）。结论 独活寄生汤联合玻璃酸钠关节腔注射在肝肾不足型膝关节骨关节炎中的应用效果较好。

Abstract:

Objective To explore the application effect of Duhuo Jisheng Decoction combined with sodium hyaluronate intra-articular injection in patients with knee osteoarthritis (KOA) of the liver and kidney deficiency type.Methods A total of 60 patients with KOA of the liver and kidney deficiency type admitted to our hospital from April 2023 to April 2025 were retrospectively selected as subjects and divided into two groups according to the treatment method: the single therapy group (n = 30) and the combination group (n = 30). The single therapy group received sodium hyaluronate intra-articular injection, while the combination group received Duhuo Jisheng Decoction in addition to the treatment given to the single therapy group. Clinical efficacy, Traditional Chinese Medicine (TCM) syndrome scores, knee joint function, Visual Analog Scale (VAS) scores, and inflammatory factors were compared between the two groups.Results The total effective rate of clinical efficacy in the combination group was higher than that in the single therapy group (P < 0.05). After treatment, the TCM syndrome scores in the combination group were lower than those in the single therapy group (P < 0.05). The Lysholm Knee Scoring Scale (Lysholm) and Hospital for Special Surgery knee score (HSS) were higher, while the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) was lower in the combination group than in the single therapy group (P < 0.05). At 3 and 5 weeks after treatment, the VAS scores in the combination group were lower than those in the single therapy group (P < 0.05). After treatment, the levels of C-reactive protein (CRP), interleukin-17 (IL-17), and interleukin-8 (IL-8) in the combination group were lower than those in the single therapy group (P < 0.05).Conclusion The application of Duhuo Jisheng Decoction combined with sodium hyaluronate intra-articular injection shows a significant effect in treating KOA of the liver and kidney deficiency type.
