Abstract:Objective To evaluate the predictive value of a model based on multivariable Lasso-logistic regression for adverse pregnancy outcomes in pregnant women of advanced maternal age with hypertensive disorders of pregnancy (HDP).Methods A total of 342 pregnant women of advanced maternal age with HDP treated at Panzhihua Central Hospital between January 2021 and June 2025 were enrolled, and adverse pregnancy outcomes were recorded. The patients were randomly divided 7:3 into a training set (n = 240) and a validation set (n = 102). Continuous variables were standardized using Z scores. A multivariable Lasso-logistic model was used to analyze the training-set data, identify factors associated with adverse pregnancy outcomes, and construct a risk prediction model. The validation set was used to validate the model. Analyses included 10-fold cross-validation, dual reporting of lambda-minimum/lambda-1-standard-error parameters, multiple imputation (m = 5), the Box-Tidwell linearity test, variance inflation factor assessment of multicollinearity, the 10-group Hosmer-Lemeshow test, calibration curves, and decision curve analysis (DCA) of threshold net benefit. Results Adverse pregnancy outcomes occurred in 62 patients in the training set and 24 in the validation set, corresponding to incidences of 25.83% and 23.53%, respectively. Lasso regression selected six variables: parity, diabetes, placenta previa, activated partial thromboplastin time, albumin (Alb), and the homeostatic model assessment of insulin resistance (HOMA-IR). Multivariable logistic regression showed that diabetes [O^R = 4.408 (95% CI: 2.507, 7.750) ], placenta previa [O^R = 3.238 (95% CI: 1.647, 6.366) ], and higher HOMA-IR [O^R = 2.617 (95% CI: 1.511, 4.533) ] were risk factors for adverse pregnancy outcomes (P < 0.05), whereas higher Alb [O^R = 0.814 (95% CI: 0.698, 0.949) ] was a protective factor (P < 0.05). The prediction equation was Logit(P) = 1.247 + 1.483 x diabetes + 1.175 x placenta previa - 0.206 x Alb + 0.962 x HOMA-IR. The Hosmer-Lemeshow test showed no significant differences between predicted probabilities and observed outcomes in either the training or validation set (P > 0.05). Calibration curves showed mean absolute errors of 0.019 and 0.036 in the training and validation sets, respectively. DCA indicated high clinical net benefit at threshold probabilities of 0.12-0.84 in the training set and 0.05-0.90 in the validation set. The model cutoff values were 230.0 and 225.5 in the training and validation sets, respectively; sensitivities were 88.7% (95% CI: 0.704, 0.912) and 87.5% (95% CI: 0.743, 0.965), and specificities were 74.2% (95% CI: 0.718, 0.901) and 73.1% (95% CI: 0.725, 0.920), respectively. Conclusion Pregnant women of advanced maternal age with HDP are at risk of adverse pregnancy outcomes associated with diabetes, placenta previa, Alb, and HOMA-IR. A risk prediction model based on these factors may facilitate early identification of high-risk patients.
